NEW Passport

Appointment required.

All applicants must be present at the time of appointment.

  1. Complete form DS-11 in black ink.
  2. Provide original evidence of citizenship. 
  3. Provide a physical photo ID.  If you present an out-of-state ID, you must present a secondary ID.
  4. Provide a passport photo.  Photo can be taken during your appointment in our Acceptance Facility for $12.  For the health and safety of your infant, you are required to bring your own white blanket for a backdrop.
  5. Personal check or money order made out to US Department of State for applicable fees.
  6. Pay City of Roseville execution fee of $35 by cash, credit card, check or money order.

Children 15 years old and under must appear with both parents.

Children 17 years old and under must have their original birth certificate for proof of parental relationship, even if they have a previous passport. The passport must be surrendered.

If your name has changed from that on the parental relationship document, you must submit proof of your legal name change (such as certified copy of a marriage certificate or court order).

If your name on a previous passport has changed , you must submit proof of your legal name change (such as a certified copy of a marriage certificate or court order).

Appointments are released each morning at 8:15 a.m., Monday through Friday, for two weeks from the day you are scheduling an appointment.

RENEW My Passport

You MUST RENEW BY MAIL if you are eligible. See the 5 eligibility requirements:

  1. Your passport is in your possession.
  2. Your passport is undamaged other than normal wear and tear.
  3. Your passport was issued when you were age 16 or older.
  4. Your passport was issued within the last 15 years, expired for 5 years or less.
  5. Your passport was issued in your current name OR you can document your name change.

If you answered YES to all of the above, please complete and follow the directions on Form DS-82.  You must then send your passport by mail through the US Postal Service.

Photos can be taken on a walk-in basis (no appointment needed) M-F, 8:30 am to 11 am and 1 pm to 3 pm.

If you answer NO to any of the above statements, DO NOT mail your renewal. You must apply in person with Form DS-11, same as a first-time applicant.  Please make an appointment through the NEW Passport option.
